Summary

The section of General Internal Medicine (GIM) is seeking a competent and reliable Licensed Vocational Nurse II to be a part of the GIM group to support our clinicians in providing services to patients. The Licensed Vocational Nurse, under supervision of the Clinic Nurse Supervisor (and practice providers) is responsible for assisting in the delivery of health care and patient care management. Candidate must possess a proactive, team-oriented attitude.  The candidate will perform nursing and administrative duties using the principles of team-based care, chronic care management, and quality improvement.  It is the responsibility of this position to contribute to a patient centric and customer focused environment where the needs of the patients come first.

 

The ideal candidate will be successful if they are organized, efficient, and self-motivated. It is very important the candidate works well with a providers, patients, and other team members.

Job Duties

  • Performs procedures as determined by the clinical needs of the department, including but not limited to intramuscular and subcutaneous injections, performing nebulizer and inhaler treatments, performing EKG's, administering and reading PPD's, removing sutures, performing ear lavage, dressing wounds, and performing other procedures that are within the scope of professional nursing practice.
  • Provides patient education related to topics such as patient self-care, taking medications appropriately, use of nebulizers and inhalers, diabetic education, wound care, and self-injections.
  • Performs and oversees non-face to face activities, including answering patient phone calls, responding to patient messages, triaging patients, and scheduling patient visits.  
  • Provides on-going assistance to support staff, MA's and supervisors in areas such as proper and thorough message taking and emergency telephone protocols.
  • Maintains awareness of clinic activity, supporting teamwork by offering assistance, and responding positively to requests for assistance. 
  • Collaborates with team members, including staff, providers and administrators of primary and other departments, to foster healthy relationships in the work environment.
  • Provides and modifies care for complex patients using clinical judgement and evidence-based analytical approach.
  • Maintains Nursing License by the State of Texas Board of Nursing guidelines and Basic Life Support BLS certification as per the Baylor College of Medicine policy.
  • Participates in patient follow-up activities, including relaying diagnostic test results, arranging for prescription refills, facilitating referrals, performing prior authorizations, and preparing information for prescription renewals at the request of the PCP/APC.
  • Performs all job functions in compliance with applicable federal, state, local and company policies and procedures.

Minimum Qualifications

  • High School diploma or G.E.D and graduated from an approved licensed practical nurse/vocational nurse (LPN/LVN).
  • Two years of relevant experience as a Licensed Vocational Nurse (LVN) or as a Licensed Practical Nurse (LPN).
  • Current license as a Licensed Vocational Nurse (LVN) or as a Licensed Practical Nurse (LPN) by the State of Texas Board of Nurse Examiners.
  • Basic Life Support certification (BLS).
