Summary
Job Purpose
The Certified Diabetes Educator (CDE) provides patient education on a diabetes diagnosis, including nutrition, medication, physical activity, and glucose testing.
Job Duties
- Provides formal diabetes education to patients in outpatient clinic.
- Provides diabetic education training to other clinical staff as needed.
- Provide nutritional information for weight control and disease management.
- Schedule appointments, tests, and examinations with hospital and other doctor’s offices.
- Document patient care services by charting in patient’s record.
- Instructs patient regarding diabetes self-management.
- Communicates with other health care professionals regarding orders and care of patients.
- Promotes patient’s independence by involving patient in defining and establishing patient care goals; teaching and counseling patient, friends, and family and reinforcing their understanding of disease, medications, and self-care skills.
- Respects and supports patient rights and promotes positive family and patient relations.
- Performs other job related duties as assigned.
Minimum Qualifications
- Bachelor's degree.
- Two years of relevant experience.
- Certified as a Diabetes Care and Education Specialist (CDCES).
