Position Summary

The Clinical Pharmacist II functions at the highest level of clinical practice and works as part of the multidisciplinary health care team to directly care for patients. As the inpatient hospital clinical pharmacist, he/she will take ownership of workflows and processes in their area of practice and help design, implement, document, and monitor comprehensive medication management (CMM) and therapeutic care plans through direct interactions with patients and providers to achieve positive patient centric outcomes. The inpatient hospital clinical pharmacist is a liaison between Pharmacy Services and Hospital & Specialty Oncology Services to support discharges and care coordination to achieve positive patient centric outcomes.

Key Responsibilities include:

Clinical

  • Provides comprehensive medication management services, coordinating all clinical pharmacy functions for the acute care areas as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
  • Completes patient profile reviews and participates in daily clinical care rounds, huddles, and meetings as an integrated member of a multidisciplinary team.
  • Reviews medication regimens for appropriateness, contraindications, and potential interactions.
  • Helps monitor therapeutic plans and medication treatment.
  • Orders, reviews, and acts upon appropriate laboratory tests and other diagnostic studies necessary to support a patient's medication therapy regimen.
  • Performs pharmacokinetic monitoring, renal dose adjustments, anticoagulant management, glycemic management, pain management, prophylaxis review, IV to PO conversions, and antimicrobial stewardship as necessary.

Education and training

  • Works directly with patients, families, and caregivers to provide comprehensive education about medications and related disease state(s). Verifies patient understanding of the medication including its intended purpose, proper use, expected outcomes, and how to obtain refills/renewals. Ensures an updated medication profile and list of medications are provided.
  • Serves as an expert on drug usage in assigned area for medical and nursing staff.
  • Uses appropriate references to research drug information.

Research

  • Develops an understanding of therapeutic clinical studies in their areas of expertise and manage patients’ treatment based on study requirements and specifications.
  • Participates in Oncology research and contribute to cancer discoveries and the clinical trials being conducted by investigators and doctors at City of Hope.

Fiscal Responsibilities

  • Engages in activities that promotes cost effective high-quality care such as review for appropriate medication use criteria, medical necessity and “on/off” label indication and coordinates necessary documentation with the ordering provider.
  • Evaluates non-formulary requests and offer alternatives to medical staff as appropriate.

Drug Information

  • Serves as an authority on drug information, usage, interactions, dosages, and compliance to medical staff, nursing staff, technicians, and students.
  • Uses appropriate references to research drug information.
  • Possesses a working knowledge of information resources.
  • Performs other related duties as assigned or requested.

Basic education, experience and skills required for consideration:

  • Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending
  • PGY1 and PGY2 residencies, or
  • PGY1 plus two years of experience in an acute care hospital pharmacy, or
  • Five years of experience in an acute care hospital pharmacy

Req. Certification/Licensure:

  • Current CA Pharmacist License or within 3 months of hire.
  • BLS (through American Heart Association) certification or receive one within 3 months of hire date.

Preferred Education:

  • Pharm D.

Preferred Courses/Training:

  • PGY-2 Residency, Board Certification.

Preferred Experience:

  • Five years of experience, preferred.
