As a Nightingale RN Care Coordinator, you will work collaboratively with multidisciplinary care teams across the continuum to facilitate effective coordination of care for high and rising risk patients. The Care Coordinator will focus efforts on patient outreach and chronic disease management for a panel of patients to promote wellness, decreasing preventable ED visits and readmissions while improving quality outcomes and patient satisfaction. Essential to this role are the core values of patient advocacy, holistic care, care planning with patient centered goals, and the empowerment of patients and families to self-manage.

Responsibilities:

  • Supports Care Coordinator panel management using their knowledge and expertise to guide/mentor those new to the role and those who would benefit from additional support in work prioritization.
  • Performs the clerical functions of the Care Coordinator or Transitional Care Management Hub Care Coordinator adding to productivity and safe patient transitions.
  • Works as part of an interdisciplinary care team.
  • Ensures compliance with regulatory standards and patient safety initiatives.
  • Other duties as assigned.

Education:

  • Graduate of an accredited school of professional nursing.
  • BSN preferred.

Certifications:

  • Current state licensure as a Registered Nurse (RN).
  • Basic Life Support (BLS) through American Heart Association (AHA).
  • Specialty certification preferred.
  • For Homecare and Hospice field staff only, must have valid Ohio Driver’s license upon hire; per policy 8-175 thereafter.
  • For Homecare and Hospice field staff only, must have proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.

Competencies:

  • Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Work Experience:

  • Minimum of 3 years’ experience in the Care Coordinator position at the Cleveland Clinic immediately prior to retirement.
  • Current demonstrated clinical competence.
  • Strong communication and interpersonal skills.

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.
  • Requires corrected hearing and vision to normal range.
  • May require some exposure to communicable diseases or bodily fluids.
  • Light work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required.
