Job Description

Summary

The Mental Health Case Worker functions as a core member of a collaborative care team that involves the patient’s primary care provider, a consulting psychiatrist, Behavioral Health Consultant and other staff member that may be available in the primary care clinic. The Mental Health Case Worker is responsible for supporting the Behavioral Health Department by providing support for and facilitating in clinic Tele-Psychiatry services to patients. He / she is also responsible for coordinating care for in clinic medication assistance treatment (MAT) patients and making any necessary referrals to clinically indicated services outside the primary care clinic (e.g., social services, mental health specialty care, and substance abuse treatment).  The Mental Health Case Worker is accountable to the Director of Behavioral Health.

Job Duties

  • Carries out the mission of Healthcare for the Homeless - Houston (HHH) and ensures the quality of services through interaction with patients. 
  • Provides support services to patients with physical health, mental health and/or substance abuse diagnoses. 
  • Builds and maintains a therapeutic relationship with patients through engagement and negotiated care plan.
  • Maintains clinical documentation.
  • Maintains organization and patient confidentiality.
  • Advocates with community service providers to facilitate access of patients to needed services. 
  • Attends and participates in meetings and Quality Improvement activities as required.
  • Participates in evaluation of peers, support staff and supervisor as required. 
  • Sustains productivity expectations established by supervisor.

Minimum Qualifications

  • Bachelor's degree in Social Work, Psychology, Human Services or a related field.
  • Three years of relevant experience.

Preferred Qualifications

  • Experience and/or interest in working with homeless and/or medically underserved populations. 
  • Ability to use Zoom, Microsoft Teams, Citrix WebEx and other video conferencing applications.
  • Ability to use Microsoft Office and Excel, spreadsheets, and computer data-base systems.
  • Practice experience in the Primary Care Behavioral Health (PCBH) model.
