As a Paramedic in our Homecare program, you will be out in the field every day, being a part of a team that strives to keep our patients safe and healthy at home, reducing the number of unnecessary trips to the Emergency Room and hospital. Your duties will include but are not limited to: • Providing home-based care that supports optimal health and wellness for the patient. • Performing assessments and reporting findings of both in-depth and focused healthcare assessments, screenings and examinations to the appropriate team member. • Providing Basic Cardiac and Advanced Life Support as outlined in existing Cleveland Clinic EMS protocols. We provide the opportunity for you to be a Certified Community Paramedic. It is required within the first 18 months of hire, and we cover the cost of the book and testing fees. Job Responsibilities:

  • Examines, screens, treats and coordinates health services for patients (may include point of care laboratory testing based on paramedic’s education, training and competency):
  • Conducts/assists with  post-hospital care including, but not limited to,  home safety/living condition evaluations, medication adherence/education, dressing changes, checking of vital signs and reinforcement of post-discharge/post-office visit education;
  • Observes, records and reports to the care team/LIP, patient’s condition, response to medications and/or treatments and any significant findings/incidents (actual /potential – Risk Assessment);
  • Conducts patient education including chronic disease management (Diabetes, Hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), fall/injury prevention, basic nutrition, post-operative wound monitoring and care;
  • Administers care consistent with program policies, standard operating procedures and LIP orders;
  • Coordinates appointments, additional home health visits  and follow-up with providers and facilities as appropriate/ordered;
  • Provides both Basic and Advanced Life Support within their scope and capabilities (equipment limitations) in collaboration with local EMS agencies. 
  • Other duties as assigned

Education:

  • Completion of accredited paramedic training program (EMT-P). 

Certifications:

  • Current certification as an Emergency Medical Technician – Paramedic in the State of practice.
  • Basic Cardiac Life Support (BLS) through the American Heart Association (AHA)
  • Advanced Cardiac Life Support (ACLS) through the American Heart Association (AHA)
  • Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter.
  • Must have reliable transportation and proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.

Competencies (Complexity of Work):

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Requires ability to work collaboratively with healthcare professionals and family/caregivers to ensure “Patients First” philosophy and achieve shared goals. 
  • Must be able to work in a residential / community environment and take appropriate action.
  • Position requires travel and driving.
  • Position is outside the normal “hospital” environment and requires working in patient’s homes/care areas. 

Work Experience:

  • At least two years of recent clinical experience as an EMT-P

Physical Requirements:

  • Requires full body motion to assist patients and practice within a home environment.
  • Ability to move and lift patients, manual finger dexterity with good eye-hand coordination, involves extensive standing and walking. 
  • MEDIUM WORK – Exerting 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical demand requirements are in excess of those for “Light Work.”

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
  • Access to Cleveland Clinic provided cellular phone and IPad.
