Pharmacist I (Inpatient)

The successful candidate:

  • Must be able to work PRN, varying shifts primarily Mon-Fri DAYS or NIGHTS, with some occasional weekends. Pharmacy is open 24 hours per day/7 days per week.

  • Responsible for reviewing and interpreting providers’ orders for all medications, screening for therapeutic incompatibilities, and ensuring cost-effective medication use that complies with the COH Enterprise Formulary.

  • Provide patient-centered care and integrated pharmacy services in a decentralized and centralized practice setting.

  • Assist with the monitoring of drug therapy, with an emphasis on chemotherapy, and is responsible for providing and educating on drug information to providers and other clinical staff.

  • Responsible for oversight of technician staff as it relates to sterile intravenous compounding, using centralized and decentralized medication-use systems, including chemotherapy and TPN, medication refilling, and unit dose drug preparations.

  • Participate in quality improvement projects with an emphasis on patient safety and will engage in multi-disciplinary initiatives as requested.

  • Serve as a resource for providers and clinical staff in a decentralized or centralized capacity, which may include answering drug information questions and assisting with navigation through EHR drug order workflow.

  • Competent in preparation and dispensing of sterile compounded products following guidelines outlined within USP 797 and USP 800, and able to supervise technician staff performing these functions.

  • Participation in departmental and hospital quality assurance programs with an emphasis on patient safety, and regulatory compliance.

Qualifications

Minimum Education Requirements

  • Graduate of an ACPE-accredited College of Pharmacy

Minimum Experience

  • Minimum of 2 year’s institutional pharmacy experience or strong interest in institutional and oncology pharmacy

Req. Certification/Licensure

  • Active license with the State Board of Pharmacy

  • Doctor of Pharmacy or other advanced degree highly preferred

Salary / Pay Rate Information:
Pay Rate: $48.61 - $80.03 / hour
