Registered Nurse - Stem Cell Transplant Coordinator

The Transplant Coordinator is a registered nurse (RN) responsible for coordinating the care of both adult Stem Cell transplant recipients and donors during the pre and post phases of the transplant trajectory.

The successful candidate:

• Coordinates evaluation process for pre-transplant patients to ensure effective use of hospital and clinical resources. This includes initiation and oversight of donor searches for unrelated donors. Responsible for initiating NMDP donor search.
• Coordinates transplantation process when a donor is available including communication between physicians, the National Marrow Donor Program as needed, patient/families and the medical team involved. Works closely with the National Marrow Donor Program to arrange unrelated donor transplants and services when needed.
• Coordinates and/or schedules appropriate pre-transplant work-up and pre-donation work-up for potential donors as directed by protocol and physician.
• Evaluates pre-transplant testing, noting discrepancies between test results and eligibility criteria. Addresses test results with physician.
• Educates patients along the continuum to assist understanding of processes and procedures surrounding pre- and post-transplant issues, disease, treatment, management, and supportive care. Educates & serves as a resource for nurses and other multidisciplinary team members about patient care issues to promote positive outcomes for transplant patients.
• Participates or leads team conferences, rounds, or meetings in relation to care of the transplant patient or program issues. Responsible to update and maintain the clinical database.
• Supports and participates in the strategic plan of COH, Department of Nursing, and Stem Cell Transplant Program.
• Completes necessary documentation according to the National Marrow Donor Program and CIBMTR guidelines including pre- and post-transplant follow-up information.
• Interacts with insurance coordinators providing appropriate data to ensure timely financial review.
• Calls patients regarding follow-up related to schedule of transplant, sibling testing, donor search, insurance transplant procedure coverage and education follow-up. Document as appropriate in the outpatient medical record.

• Current IL Registered Nurse license
• Bachelor's degree in Nursing (BSN)
• Basic Life Support (BLS) through American Heart Association
• ACLS through American Heart Association
• 3 to 5 years nursing experience
• Completion of ONS Chemotherapy and Biotherapy class preferred.
• Experience with oncology, specifically hematology and stem cell transplant.
• Strong organizational skills to coordinate patient care and prioritize appropriately; detail oriented
• Computer skills including electronic health record, Outlook, Word, Excel
• Good verbal, written, and interpersonal skills to support customer relations and communicate with other health care providers, employees, and the public.
• Strong critical thinking skills for donor procurement, education, and outcomes
• Ability to organize and be self-motivated
