The successful candidate:

Provides comprehensive clinical and psychosocial assessment and intervention for patients with serious illness. Works independently and alongside other members of the multidisciplinary care team to conduct and participate in family conferences, particularly when there are complex psychosocial needs. Consults and educates staff regarding social work practice, palliative care and end of life issues. Provides clear communication, both written and verbal, to the multidisciplinary staff regarding patient care concerns and end of life issues. Collaborates with appropriate clinical staff regarding patient and family centered discharge planning; identify high-risk psychosocial factors of patient/families that impact health status and decision making.

JOB RESPONSIBLITIES

  • Works collaboratively and maintains active, open communication with physicians, patients, families and other healthcare team members including external health care agencies to facilitate the plan of care in a courteous, professional manner.
  • Calls patients/families, as needed, after discharge to the clinic to ensure plan of care discussed results in a smooth process and proactively identifying obstacles that may delay treatment progress. Follow up with patients/caregivers, as needed, to monitor the success of the discharge plan and proactively identify barriers that may delay treatment.
  • Documents all interventions in the electronic health record in a timely manner.
  • Addresses needs of both patient and family through counseling, education and community resource linkage serving as an advocate for patients/families in regard to decisions affecting the plan of care.
  • Advocate for clients or patients to resolve crises.
  • Facilitate referrals to external providers i.e. home health care, durable medical equipment providers, skilled nursing facilities, acute rehab, and hospice providers as necessary.
  • Collaborate with multidisciplinary team to facilitate safe and efficient patient discharge from the inpatient unit.
  • Assesses depression and anxiety in pre/post-surgical patients per ASURE protocol.
  • Completes psychosocial assessments/discharge planning screens on new inpatient admissions as well as intake patients as referred.

JOB QUALIFICATIONS

Education: Master of Social Work from a program accredited by the Council on Social Work Education

Licensure: Licensed Master Social Worker (LMSW)

Salary / Pay Rate Information:
Pay Rate: $27.76 - $45.67 / hour
